Your Options for Providing Medical History

Online Form

Available 24 hours a day, 7 days a week

When you proceed through the payment steps of your order online, you will be prompted to input your medical history on our online form. This information will be transmitted directly to our pharmacists.

If you have already provided your medical history to us (via online form, phone call or in-person), and there has been no changes since that time, you have the option to select “No” when prompted “Has anything about the medical history of (your name) changed since the last time you ordered medicine from us?”. This means you only need to answer our questions for your first order, or if there have been any changes to your medical history.
